Included are forms for both Coaches and Players. These forms must be
completed in their entirety and be included in the Team Binder.

A standard form of release required to be completed in its entirety by every
player and then inserted into the Team Binder. Please note that members of
The 949 Roller Hockey Center must also complete their Release
Form but that this is separate from the IHF forms and is not a
requirement for including into the Team Binder.

Combo Players and Teams

IHF Combo Player Registration form (download the
Word.doc). This is a Registration form intended for use by a Player to be registered into the prospect pool for this new division. Please complete the form and email it to the league as instructed, or by fax. The League and rink partners will soon be reviewing all registrants to then contact and help to match with a team near you.
Spring Season

Rules,
Player eligibility, forms and procedures will be a match to the IHF Fall/Winter
Season. Tryouts and practice slots, fees and scheduling are arranged exclusively
by The 949 or the rink of your choice. Teams who participated in the fall/winter
league may reuse the Team Binder - just complete a new Spring Roster form replace and/or update new player information. Get

*About Team Binders
It is the responsibility of each team to complete the requirements and contents
for a Team Binder as defined in the IHF Rules**. Teams will assemble the
content from the required forms and as described in the Rules to then produce a
binder. The completed materials will be checked by IHF officials at an
appointed time and in advance to the first regular season game of each season.
The IHF League will provide the appropriate 3-ring binder at check-in, and once
completed and approved, remains the property of the IHF and maintained at the
League's home rink, The 949 Roller Hockey Center. It is recommended that
all Teams make a copy of the binder and contents for use when travleing to other
rink facilities and for registration in high school (scholastic) divisions at
sanctioned tournaments.
** SECTION III. PLAYER ELIGIBILITY
2. Every team in each division must submit a separate Team Binder by the date directed by the IHF Board of Directors for that season. The master team binder will be maintained at the primary facility for League reference at all times. A Team Binder copy, black & white acceptable, must accompany each team and be made available to officials at each roller hockey facility where the team may travel and play league game(s). All Team Binders will be 1 1/2”, white 3-ring and color coded on the binder’s spine by Division. The IHF will provide each team with a Binder and produce a color-coded label at time of Check-In as the Board may direct. Color codes by division are:
1. Varsity =
Black;
2. JVA =
Blue;
3. JVB =
Red;
4. JVC =
Green; and
5. Combo =
Yellow
The Team Binder(s) must contain:
1. Official IHF Team Profile form (must be typewritten);*
2. Official IHF Final Team Roster (typewritten);*
3. Completed IHF Conduct of Coaches Form for each coach;**
4. Legible color copies of each student’s current school ID, enlarged to fit on an 81/2 x 11" page, with a photo that clearly shows the student’s face;**
5. Completed IHF Liability Release Form for each player;**
6. Completed IHF Player Conduct Form signed by Player and/or Parent of minor;**
*Must be completed and returned to the IHF by email and placed in Team Binder.

About Checking In Your Team
All Players and Coaches from all teams must be entered into The 949 database to be
included on any Game Scoresheet - this due to the elimination of the Game Roster
form. Team Binder information and Forms are described above. Final Rosters are due to the League in digital form before the 3rd preseason game. Binder check-ins for 2010-11 will be by Appointment Only. Binders and all content must be completed in their entirety in advance to the first regular season game. Binders must include: IHF Team Profile form; Final Team Roster; Code of Conduct for Coaches; Code of Conduct for each Player; Liability Release Form for each Player; Legible color copy of each student’s current school ID; AAU Insurance number and/or copy of membership for each Coach and Player; and Certification number and/or copy of NFHS Training Certificate for each Coach.
